Diltiazem hydrochloride powder for injection is the generic form of the brand-name drug S.Q-Dilzem, a drug indications Supraventricular tachycardia, Emergency treatment of abnormal hypertension during surgery, Hypertension emergency, Unstable angina

Side effect:

  • Common adverse reactions were: bradycardia (1.1%), hypotension (0.7%), degree I atrioventricular block (0.4%), degree II atrioventricular block (0.3%), atrioventricular junctional rhythm ( 0.3%) etc.
  • Occasionally complete atrioventricular block, severe bradycardia (initial symptoms: bradycardia, dizziness, mild headaches, etc.), and sometimes can lead to cardiac arrest, need to be well prepared to deal with these symptoms after the start of medication. If abnormality occurs, medication should be discontinued immediately and the following treatments should be performed: Complete atrioventricular block, severe bradycardia: atropine sulphate, isoproterenol, etc. and/or use of cardiac pacing. Stops the heartbeat: Cardiac resuscitation is given to cardiac massage and catecholamines such as epinephrine.

Warning:

  • Patients with severe hypotension or cardiogenic shock are disabled
  • II and III degree atrioventricular block or sick sinus syndrome (sustained sinus bradycardia (heart rate less than 50 beats / min), sinus arrest and sinus block, etc. are disabled
  • Patients with severe congestive heart failure are disabled
  • Patients with severe cardiomyopathy are disabled
  • Allergic to any ingredient in the drug are disabled
  • Pregnant or possibly pregnant women are disabled
  • Other antiarrhythmic drugs (dappapine phosphate) were combined with terfenadine, and there have been reports of prolonged QT interval and ventricular arrhythmias.
  • Intravenous administration of diltiazem and intravenous β-blockers should be avoided within the same or similar time (within hours).
  • In patients with ventricular tachycardia, the use of calcium channel blockers in patients with wide QRS tachycardia (QRS ≥ 0.12 s) may exhibit hemodynamic deterioration and ventricular fibrillation. Before intravenous administration of diltiazem, it is important to determine whether the wide QRS complex is supraventricular or ventricular.
  • For severe attacks with angina attack lasting more than 15 minutes, it is necessary to consider other treatments (PTCA, CABG, etc.).
