Side effect:

  • Theophylline toxicity often appears in serum concentrations of 15 ~ 20μg/ml, especially in the beginning of treatment, early common nausea, vomiting, irritability, insomnia, etc.
  • When the serum concentration exceeds 20 μg/ml, tachycardia and arrhythmia may occur.
  • Theophylline in serum exceeds 40 μg/ml, which can cause fever, dehydration, convulsions and other symptoms. Severe respiratory and cardiac arrest can cause death.

Warning:

  • In patients with allergies to this product, patients with active peptic ulcers and uncontrolled convulsive disorders are disabled
  • Like other theophylline sustained-release preparations, this product is not suitable for patients with persistent asthma or acute bronchial spasm.
  • Serum theophylline concentration should be monitored regularly to ensure maximum efficacy without the risk of excessive plasma concentrations.
  • Theophylline preparations can cause arrhythmia and/or worsen the original arrhythmia; any changes in the patient’s heart rate and/or rhythm should be monitored and studied.
  • Use this product with caution in patients with a history of hypoxemia, hypertension, or peptic ulcer

Interactions:

  • Diltiazem, verapamil can interfere with the metabolism of theophylline in the liver, combined with this product to increase the blood concentration and toxicity of the product.
  • Cimetidine can reduce the liver clearance rate of the product, combined use of theophylline can increase the serum concentration and (or) toxicity.
  • Some antibiotics, such as macrolide erythromycin, roxithromycin, clarithromycin, quinolones enoxacin, ciprofloxacin, ofloxacin, levofloxacin, clindamycosis Sucralose, lincomycin, etc. can reduce the clearance of theophylline and increase its plasma concentration, especially erythromycin and enoxacin. When theophylline and the above drugs are used, they should be reduced appropriately.
  • Phenobarbital, phenytoin, rifampicin can induce hepatic drug enzymes, accelerate the hepatic clearance rate of theophylline; theophylline also interfere with the absorption of phenytoin, both plasma concentrations are reduced, the dose should be adjusted when combined.
  • Combined with lithium salt, can increase the renal excretion of lithium. Affect the role of lithium salt.
  • In combination with mexiletine, can reduce theophylline clearance, increase plasma theophylline concentration, need to adjust the dose.
  • Use with caffeine or other xanthines to increase its effect and toxicity.
